CMSST is looking for 8 companies to sponsor their Attendee Conference Bags. The cost for sponsorship is $500 payable no later than April 26, 2008. VISA or MASTERCARD will be accepted for payment.

• Be printed on the bag.
• Receive verbal recognition at the conference, and
• Appear in the syllabus
• Be placed on the CMSST Website as a 2008 Sponsor.

Rules of Special Sponsorship:

• All sponsorships are on a first come, first serve basis.
• Company categories constitute ONE OF EACH:
  • DME,
  • HOME HEALTH,
  • REHAB,
  • LONG TERM CARE/SNF,
  • HOSPICE,
  • IV INFUSION,
  • LTAC,
  • ASSISTED LIVING.

Winners will be contacted via email.

The form should include the following:
Name of company, address, phone, fax number, Name of contact with their work cell number. The manner in which they want to make the payment Check or Credit Card.
